Family Thank you for visiting the MPGN Survey. The MPGN Survey is a research project conducted by a team of nurses, informatics specialists, and pediatric nephrologists at The University of Iowa. The purpose of this survey is to collect information from individuals with MPGN Types I, II, and III. This information will give us clues as to a cause, cure, and the best possible treatments for people with MPGN. You are a valuable part in finding answers to this disease.

Request to Participate

If you are over age 18 and have biopsy-proven MPGN or are the parent or legal guardian of a child under age 18 who has biopsy-proven MPGN, use the Request to Participate form to sign up for the survey and receive a password that will allow you to sign in and complete the survey.

  1. Who can participate in this study?
    If you have biopsy-proven MPGN I, II, or III and are over the age of 18 you can fill out the survey. If you are the parent or legal guardian of a child under the age of 18 you can fill out the survey on their behalf.

  2. How do I fill out the survey?
    You can complete the survey directly on the web or you can request a paper copy. To access the survey on the web use the Request to Participate form to obtain an id and password. If you would like us to send you a paper copy of the survey, you can contact us using the web contact form by fax at 319-353-5326 , or phone us at 319-335-7104.

  3. How will the University use my data?
    Please see the terms and conditions of the study for more information on how your data will be used and protected.

  4. Will my name appear in any report of this study?
    No. Your name will not appear in any reports of this study. Only the members of the research team will have access to your name and answers.

  5. Why should I participate in the study?
    The results of this study are yielding new and more detailed knowledge about MPGN. This information is available to doctors and other professionals through professional publications
